Since founded in 1998 as a home visiting program, we have been able to provide much-needed health and social services to families from every lifestyle.
What We Do
Healthy Connections, Inc provides the following services:
In 2007, Healthy Connections received a new grant to open another community health center in Mt. Ida-the Montgomery County Community Clinic. M.C.C.C. opened in December of 2007 and is a Federally Qualified Health Center providing primary and preventative medical services. The Clinic also provides diabetic services (DIMES program) and dental services.
